3650 Route 112, Coram, NY 11727
(631) 345-6670
Route 112 runs through the middle of a stretch of eastern Suffolk County that does not always make the list when people talk about Long Island healthcare access. Coram, Middle Island, Ridge, Rocky Point – these are working communities spread across the Town of Brookhaven, far enough from the hospital corridors of Nassau County that getting to a good doctor has not always been simple.
That is what Heart and Health Medical’s Coram office was built to fix.
Located at 3650 Route 112, the Coram location brings a full roster of board-certified physicians and specialists to a part of Suffolk County that needs exactly that. Cardiology. Family and internal medicine. Podiatry. Allergy and immunology. Primary care. All under one roof, with the same standards of care that have made Heart and Health Medical a recognized name across Long Island since 2005.
Heart and Health Medical has been voted the best of Long Island for providing exceptional medical care by our doctors in Allergy & Immunology, Cardiology, Internal Medicine, Family Medicine, Podiatry. We have been recognized by our patients as well as the Town of Babylon, Town of Oyster Bay, State of New York, Nassau & Suffolk County Legislators.
At Heart and Health Medical, our patients are part of our extended family. We strive to make every patient feel comfortable and appreciated while providing the most advanced medical care possible. The doctors of Heart and Health understand that every patient is an individual and needs a medical care plan that is as unique as they are.

Heart and Health Medical is the only private physician-owned group medical practice in Long Island. Heart and Health Medical accepts most insurances.
The Coram office is located on Route 112 – also called Coram-Middle Island Road – in a section of the road that is the commercial and medical hub for this section of Brookhaven Township. Coram is connected to the Long Island Expressway (I-495) at Exit 64 – this allows access from all over central and eastern Suffolk County, avoiding those secondary routes.
Those arriving from Middle Island travel about five minutes south on Route 112. Patients from Ridge and Rocky Point drive south on Route 25A or local roads until they reach Route 112 south to Coram. From Yaphank, the LIE west to Exit 64 makes for the fastest route. Those from Farmingville, Lake Grove and Centereach travel to the office on Route 112 heading north.
The 3650 Route 112 office has parking spaces. The office is open Monday to Friday 9:00 am to 5:00 pm.

Heart and Health Medical’s Coram location has one of the most complete provider rosters of any of the practice’s five Long Island offices. Patients do not need to travel to Massapequa or North Babylon for specialist care – most of it is available right on Route 112.
Dr. Kavesteen is the founder of Heart and Health Medical and the practice’s lead cardiologist. He trained at SUNY Stony Brook, completed his MD at SUNY Health Science Center at Brooklyn, his Internal Medicine residency at New York University Medical Center, and his Cardiovascular fellowship at Maimonides Medical Center. He is recognized as a Leading Physician of the World by the International Association of Cardiologists and named One of America’s Top Cardiologists 2010 by the Consumers’ Research Council of America.
He sees cardiology patients at the Coram location. For Suffolk County residents who have wanted access to physician-founded, physician-led cardiac care without driving into Nassau County, his presence in Coram specifically is worth noting.
Dr. Farahmandpour has been seeing patients at the Coram office for years – long enough that some patients have been with him for two decades. That kind of tenure means he knows his patients as people, not just as charts. He graduated cum laude from Hofstra University and completed his DO at New York College of Osteopathic Medicine in Old Westbury. His residency at Long Beach Medical Center was followed by a faculty appointment at NYCOM as Assistant Professor in the Family Medicine Department.
His background in osteopathic medicine gives him a clinical framework that addresses musculoskeletal issues – headaches, lower back pain, arthritis – through hands-on techniques as well as medication when appropriate. Patients looking for a family medicine doctor in Coram who will actually sit with them and work through a problem will find that in Dr. Farahmandpour.
Patients who have been seeing him at Coram describe him consistently as someone who does not rush appointments and explains things clearly. One long-term patient noted he is the doctor who will sit and listen rather than rush you out the door. Another, who has been coming to the Coram office for over 20 years, called him very competent, responsive, and thorough.
Dr. Gutierrez received his medical degree from St. George’s University School of Medicine and has practiced internal medicine for more than 20 years. He is affiliated with Plainview Hospital and Syosset Hospital. His approach – listening and caring for each patient as a whole rather than treating symptoms in isolation – comes through consistently in how his Coram patients describe their visits.
One patient who came to the office for the first time after years of avoiding doctors described how Dr. Gutierrez had read the chart before walking in, focused on the most urgent concerns first, made a concrete plan, and ordered blood work and referrals in the same visit without pushing unnecessary extras. For anyone who has been putting off primary care and is coming in with accumulated issues, that kind of organized, unhurried approach matters.
Dr. Hansen completed her undergraduate degree in Biochemistry at SUNY Stony Brook, her Doctor of Podiatric Medicine at New York College of Podiatric Medicine and her Podiatric Surgical residency at the VA Medical Center in Northport – where she also worked in wound care at Stony Brook University Hospital’s Leg and Foot Ulcer Group. She’s a member of the American Podiatric Medical Association and a Podiatric Surgical Attending at Southside Hospital in Bay Shore.
For Coram patients dealing with foot pain, nail conditions, diabetic foot concerns or anything else in the podiatric scope, Dr. Hansen brings surgical training and clinical depth that goes well beyond routine care. Patients describe her as taking her time to understand the problem before treating it – a detail that matters when a condition has been ongoing.
Dr. Guida trained at St. George’s University School of Medicine, completed his pediatric residency at Monmouth Medical Center, and went on to two fellowships – Cystic Fibrosis and Pediatric Pulmonology at Hahnemann University Hospital in Philadelphia – followed by Adult and Pediatric Allergy and Immunology at Nassau County Medical Center. He’s Medical Director of the Adult and Pediatric Cystic Fibrosis Programs and the Family Asthma and Allergy Center at Good Samaritan Hospital Medical Center in West Islip, and an adjunct clinical assistant professor of Pediatrics at New York College of Osteopathic Medicine.
He is a Fellow of the American Academy of Allergy, Asthma and Immunology, the American College of Chest Physicians, and the American Academy of Pediatrics.
For Suffolk County patients managing asthma, environmental or food allergies, or complex immune conditions, Dr. Guida brings fellowship-level specialist depth to the Coram office. His presence here means allergy and immunology care along the Route 112 corridor without a long drive into Nassau County.
EKGs, blood pressure, and heart checks with Dr. Kavesteen. This is a potentially useful service to those from Rocky Point, Ridge, and Middle Island needing access to a cardiologist in Suffolk County without having to venture into Nassau.
Annual exams, sick visits, preventive screenings, management of chronic medical conditions, and immunizations with Dr. Farahmandpour, Dr. Gutierrez, and Krystie Manaro, NP.
Dr. Sam Benyamin, PT, DPT, joins Coram with his Doctorate in Physical Therapy from Stony Brook University. He’s a specialist in orthopedic, sports, and neurological rehabilitation – including spinal mobilizations, myofascial release, Graston technique, and blood flow restriction (BFR) training.
Foot and ankle examination, diabetes foot care, nail diseases, wound care follow-up with Dr. Hansen.
Allergy testing, treatment plan, immunotherapy and asthma/immune conditions with Dr. Guida.
Adult medicine with all aspects of care with Dr. Gutierrez.
Same-day, walk-in care for acute, non-emergency medical issues. Open during office hours. No appointment necessary.
The patients who come to 3650 Route 112 are drawn from nearby communities. Coram and the Brookhaven corridor – Middle Island, Ridge, Rocky Point, Yaphank, Farmingville, Lake Grove, Centereach – attract residents who have previously had to travel west into Nassau County or north to Port Jefferson for access to specialists. The Heart and Health Medical Coram office was the practice’s answer to that gap in eastern Suffolk.
Some patients have been coming since the office first opened. One long-term patient reviewed the location after more than ten years without a single change in practice. Another described over 20 years with Dr. Farahmandpour at this specific office. What sustains that kind of loyalty is not one thing – it is the fact that the office runs consistently, the staff knows patients by name, and the wait does not swallow an afternoon.
New patients arriving for the first time after years away from regular care, or those who have recently moved to eastern Suffolk and need to establish care with a practice that actually has specialist depth, find in the Coram office something that is harder to come by in this part of Long Island than it should be: a single address that handles cardiology, allergy, podiatry, and primary care without sending them somewhere else for each.
Heart and Health Medical accepts most major insurance plans at the Coram office, including Medicare, Medicaid, Aetna, Cigna, United Healthcare, United Healthcare Community Plan, Empire, Humana, Oscar Health, GHI, Oxford, MagnaCare, Affinity Health Plan, and others.
