Requesting a prescription refill is easier when you have the right information ready. At Heart & Health Medical, we want to make the refill process as convenient, accurate, and safe as possible.

Basic Information to Share

To help us locate the correct medical record and avoid delays, please provide:

• Your date of birth
• The best telephone number to reach you
• The name of your Heart & Health Medical provider

Having accurate information allows our staff to locate your medical record and route your request to the appropriate provider.

Your Medication Information

Please have your medication bottle or prescription information available whenever possible.
We may need:

• The exact name of the medication
• The medication strength or dose
• How often you take the medication
• The prescription or Rx number, if available

If you are unsure about the name or strength of your medication, check the prescription label or contact your pharmacy.

Your Pharmacy Information

Please provide:

Many pharmacy chains have several locations in the same area. Providing the exact location helps prevent your prescription from being sent to the wrong pharmacy.

When Will My Prescription Be Ready?

However, processing times may vary depending on the medication, provider availability, pharmacy requirements, insurance authorization, and whether additional medical evaluation is necessary.
Please do not wait until your last dose to request a refill.

Important Prescription Refill Policies

Your safety comes first. Medication refills are not simply administrative requests; your provider may need to determine whether continuing a medication remains appropriate and safe.

Patients Who Have Not Been Seen Recently

Certain medications require regular medical follow-up. If you have not been evaluated within the timeframe recommended by your provider or Heart & Health Medical, you may be asked to schedule an appointment before additional refills are authorized.
For many patients receiving ongoing prescription medications, an appointment may be required if they have not been seen within the previous three months.
This allows your provider to review:

Controlled Substances

Controlled substances are subject to additional federal and New York State prescribing requirements.

Depending on the medication and circumstances, controlled-substance prescriptions may require a new prescription and appropriate clinical evaluation rather than a routine refill. Electronic prescribing requirements and other restrictions may also apply.

Antibiotics and Other Medications Requiring Evaluation

Antibiotics are generally not renewed as routine prescription refills. A new or persistent infection may require medical evaluation before an antibiotic can be prescribed.
Certain other medications may also require an office or telemedicine visit, laboratory testing, or additional clinical information before they can be safely renewed.

90-Day Prescriptions

When medically appropriate and permitted by your insurance plan and pharmacy benefit, your provider may prescribe up to a 90-day supply of certain maintenance medications.

Not every medication is appropriate or eligible for a 90-day supply.

What If I Am About to Run Out of Medication?

If you are running low on an important medication, contact Heart & Health Medical as soon as possible.

Do not stop an important prescription medication suddenly unless instructed to do so by a qualified healthcare professional, because abruptly discontinuing certain medications can cause significant medical problems.

Depending on the medication and circumstances, your provider may determine that a limited emergency or bridge supply is medically appropriate while you arrange the required appointment.

Your pharmacist may also be able to advise you about emergency-supply options permitted under applicable law and pharmacy policy.

Controlled substances, antibiotics, and certain other medications may not be eligible for an emergency or bridge supply.
