Affinity Health Plan has been offering New York State-sponsored free or low-cost health coverage to New Yorkers who have Medicaid or no health insurance at all. Building on this unparalleled success, we added Medicare Advantage to seniors and dual eligible. Our goal is to help our Members stay healthy. Each of our Members can count on our promise of great customer service. We promise to work hard to make it easy to get the care you need.
Our managed care programs meet the need for coverage among people who are "underserved," those who may find it hard to get health care for different reasons such as language barriers, social and economic status or other factors.
We are always looking for new and better ways to improve your health care. We work with over 26,000 primary care and specialty doctors and other health providers to make sure our Members get the services they need. We also give our Members information to help them seek care wisely. At our Community Service Centers, you can learn more about and get help with many different types of health programs and other free services.
Affinity Health Plan care management programs. These programs are designed to:
- care for pregnant women and their newborns
- make sure children get the shots they need
- help people with illnesses such as asthma, diabetes, and HIV/AIDS
We know that we cannot achieve our mission by acting alone. It is important for Affinity to work with other organizations that share our goals. We are continuing to build strong collaborations with community health centers, local leaders, faith-based groups, schools and area businesses. Our strong nework of doctors, community collaborators, neighborhood centers and a dedicated staff who speaks your language put us in a great place to care for you and your community like no one else.
