Covid-19 Vaccinations & Antibody Testing

Physical Exam, Blood Pressure, EKG, Blood Test (including but not limited to COVID-19 Antibody Titer), Body Oxygen Saturation, and Vaccination (Moderna). If you have already had your annual physical for 2021, an office visit will apply. All copay & deductible will apply as per your medical insurance. The second vaccination appointment will be performed at the Heart and Health of Massapequa office.
